Post Acute Disposition Provider Table
Data tables show which types of post-acute care, if any, were utilized following patients’ initial discharge from the hospital from the most recent data year and ALOS, mortality and readmission information for each segment. Data is further broken down to focus on end-of-life patients.

Connection from the Hospital to the Post-Acute System
Patient flow data is presented for different types of post-acute facilities. Which hospice providers, skilled nursing facilities, home care agencies, long term care hospitals and rehabilitation hospitals patients are discharged to are included along with the percentage of discharges into each different facility by provider type both for all initial inpatient discharges and for discharges for end-of-life patients.
