How we collect, use and store information:

We collect certain information that site visitors may provide to HealthPivots. This information includes all fields on our Order Form and Contact Us form. Order information is used in the development, follow-up communication and delivery of Atlas products and is retained in order to notify past customers about the availability of updated publications, or to contact customers regarding satisfaction with their experience. Contact information is used to reach out to individuals who have indicated an interest in specific HealthPivots products. We do not share any of our customer information with third parties.

Information about you we may receive from third parties:

HealthPivots has strategic partnerships with a limited number of agencies and individuals. Where there is a business relationship such as a third party selling HealthPivots’ products, HealthPivots receives and collects all customer information needed to fulfill orders. HealthPivots reserves the right to notify customers about new products or to follow up to assess customer satisfaction.

Analytics information:

HealthPivots may collect analytics data, or use a third-party analytics tool, to measure website traffic and assess usage. These tools may collect information such as browser type and version, device type, and user location. This information is collected and used only in aggregate form and HealthPivots does not seek to identify information about any individual site visitor.

HealthPivots may collect Traffic Data through the use of “Cookie” technology. “Cookies” are data files sent to and stored on the computer you use to view a web site. “Cookies” can be used to monitor use of our Site, to customize content specific to your interests, to ensure that you do not see the same advertisement repeatedly and, if we adopt a registered user system, to store your user name and password so you do not have to re-enter it each time you visit the Site. You can, of course, set your Web browser to prompt you before you accept a cookie, accept cookies automatically or reject all cookies.

HealthPivots may link to other websites. These may include agencies with whom HealthPivots is a strategic partner, agencies that represent the different sectors of the HealthPivots client base, sources of post-acute health system news, and/or agencies that collect data used in our products. HealthPivots is not responsible for the content of these websites. Browsing on these websites is subject to the rules and policies of these websites.

Health data:

HealthPivots follows the rules and regulations specified by the Center for Medicare and Medicaid Services (CMS) regarding the use of the Limited Data Sets (LDS) and CMS’ Chronic Conditions Data Warehouse (CCW) Virtual Research Data Center (VRDC). In the datasets provided by CMS, all patient identifiers are pre-encrypted. In compliance with the CMS Data Use Agreements regarding the LDS and VRDC, HealthPivots does not report patient counts less than 11, nor figures that would permit a computation of patient counts less than 11.
